Police are found negligent in the investigation of teen hockey player Ben Teague

Ben Teague, in September of 2019 met up with 13 teammates for a team building exercise that sadly ended up in the death of the youth hockey player.

The weekend started for the Oakville Rangers Midget Double-A hockey team at a nearby YMCA and tragic struck before the weekend was over. Now, police officers in York, Ontario have been found negligent in the investigation of the incident.
The parents of Ben Teague opened multiple lawsuits in 2022 against the Rangers, the coaches, the YMCA and the Ontario Minor Hockey Association. Later on, in 2023 they filed a complaint with the Office of the Independent Police Review Director. Their complaint was against the York police for not properly investigating the death of their son.
The review was completed in October of 2023, but just now made public. It was found that the York Regional Police officers were found negligent for not isolating or even conducting interviews with witnesses.
During the night in question, Teague and some of his 16- and 17-year-old teammates were drinking. Teague had took his coaches this the next day after experiencing excruciating leg pain.
"...a call was placed to 911 and Ben was taken by ambulance to Brampton Civic Hospital where he was pronounced dead on Sept. 14, 2019, at 7:07 a.m.

...the cause of Ben's death was determined as acute myocardial injury with necrosis associated with recent alcohol consumption."
The coaches involved have stated that the Oakville Rangers have a very clear and strict policy for alcohol, with zero tolerance. They insist all the players knew this. They also have stated that they "provided reasonable and appropriate supervision to the players, taking into consideration the age and experience of the players."
TSN/CTV Rick Westhead has said it is not clear as of yet if any of the four officers found of negligence have been disciplined, and if so, if that information would be available or made public.
An independent investigation is being done on the coaches on behalf of Hockey Canada.
